2. Discuss ways to effectively assess a chronically ill patients’ health literacy
3. Explain how the concepts used in complex case management can assist healthcare professionals navigate through the 2030 problem
4. Recall the importance of engaging with the patients’ current healthcare team
5. Demonstrate how to educate patients in goal setting
6. Discuss the concepts of resiliency, de-escalation and triangulation
7. Recognize the signs and symptoms of burnout
